Assertive Community Treatment (ACT)

Assertive Community Treatment (ACT) provides practical, everyday help for people living with serious mental illness.

ACT is for those who: 

  • Are 18+ and suffer from schizophrenia or schizo-affective disorder.

  • Repeated hospitalizations or jail.

  • Are struggling day-to-day with basic tasks like showering or getting dressed.​

​

Trained professionals stay by your side, helping you manage your mental health through consistent, whole-person care.  

​

  • 24/7 crisis support

  • A prescriber to help manage your medication

  • A therapist who understands your needs

  • A nurse to help with medical problems

  • Support from a peer who’s been there

  • Help with housing, jobs, and daily needs

  • Flexible visits in the community, shelter, at home, or at LifeHouse
